Providing quality hospice care requires a team of dedicated professionals working together to meet the physical, emotional, and spiritual needs of each patient. Hospice services typically include physician and nursing care, hospice aide/homemaker support, social work and counseling, durable medical equipment and supplies, and medications related to symptom management, depending on the individualized plan of care and payer coverage. The plan of care is individualized and directed by the attending physician and hospice team, and we involve family members as partners in care.
We provide compassionate support with essential everyday tasks, including bathing, dressing, eating, toileting and personal hygiene, incontinence care, and safe bed transfers and repositioning.
Our hospice team works with the attending physician to support pain and symptom management using appropriate medications and other comfort-focused approaches, based on the patient’s individualized plan of care.
Licensed nurses deliver skilled medical and nursing care under the direction of the patient’s attending physician, ensuring clinical needs are met with expertise and compassion.
As appropriate and available, we may offer supportive comfort measures such as relaxation, gentle touch, music, or aromatherapy to enhance comfort and reduce stress. Availability may vary and may not be covered by all payers.
When appropriate and available, we may help support condition-appropriate engagement and social connection as part of a holistic approach to comfort.
