• Inspect and get a copy of your health information
• Request correction/amendment
• Request confidential communications
• Request a restriction on certain uses/disclosures
• Get a list (accounting) of certain disclosures
• Choose someone to act for you
• File a complaint without fear of retaliation
• Receive notice of certain privacy breaches
